Together with health care providers, we’re closing gaps in care so every patient receives the right care at the right time.
Partnering to Improve Equitable Access to High-Quality Care
Susan G. Komen® partners with hospitals, cancer centers and care teams to integrate trusted navigation, education and support resources into clinical practice – so every patient receives the right care at the right time. We work hand in hand with health care providers to strengthen systems of care and ease burdens for clinicians and staff by:
- Expanding Access: Offering national, virtual patient navigation and financial assistance to complement on-site navigation programs. Personalized and bilingual navigation addresses social, financial and emotional barriers to care, filling workforce and resource gaps.
- Enhancing Quality: Supporting value-based care and accreditation standards (CoC, NAPBC, CMS) through data-driven, equity-focused interventions and education.
- Building Workforce Capacity: Providing free, standards-based training to improve supportive care for a range of professionals.
- Sharing Best Practices: Translating lessons from national quality improvement (QI).
Shared Challenges in Today’s Landscape
Despite progress, too many individuals still face late-stage diagnoses, limited access to care and inadequate navigation support – challenges that disproportionately affect under resourced communities. Across the health care ecosystem, organizations are striving to deliver high-quality, equitable care but often lack trusted, culturally relevant tools and resources to support patients throughout their breast cancer experience. At the same time, they face increasing pressure to improve outcomes, reduce disparities and meet population health, workforce development and value-based care goals.
Komen offers practical, evidence-based solutions designed to help organizations meet these challenges – supporting measurable improvements in equity, quality and outcomes across the continuum of breast cancer care.
Delivering Value Through Partnership
Komen partners with organizations across sectors to close gaps in care so every patient receives the right care at the right time. Our mission-aligned programs help partners advance value-based care and the goals of the Quintuple Aim – advancing health equity, improving population health, reducing costs, enhancing patient experience, and improving clinician and staff well-being.
